If the form doesn't work for you, send the info via OckGal[at]comcast[dot]net (subject: Max Affiliates). I put most affiliate buttons on my main page (and all of them on this page) and would appreciate my site's button be placed somewhere fairly noticeable (not a links page, otherwise that's just a link exchange) in return. If a person has to go through several pages to get to it, then it's not exactly fair for your site to be listed on my main page and mine hidden somewhere on your site.
